Adult Day Health Services

The Easterseals Adult Day Health Services program serves adults 18 years of age and older who want to experience life, relationships and fun activities in a community setting rather than spend the day at home or in a nursing facility. 

Participants in the program can continue to live at home while spending the day in a caring, supportive and active social environment. The program also provides a break for caregivers, spouses, adult children and others who have the dual responsibilities of working and taking care of a loved one. Within the program, young-at-heart participants of all ages value their independence and control over the activities they choose to enjoy.

Easterseals specializes in working with both

younger and older adults with acquired disabilities such as:

  •  Multiple Sclerosis

  •  Stroke

  •  Brain or spinal cord injury

  •  Orthopedic injuries

  •  Traumatic brain injuries

  •  Those who need some level of support in day-to-day activities due to aging/dementia

You get individual care.

An individualized plan of care is developed that will provide for the participant’s physical, social and emotional needs in a safe and caring environment. 

You are in control. 

Easterseals encourages families and participants to exercise self-direction within the program. Participants decide the activities that they choose to do, including arts, crafts, current event discussions, exercise, group games, computer access, community experiences and more!

We have a flexible attendance schedule, tailored to fit your needs. Participants may attend up to 5 days per week.

At Easterseals, we promote well-being through

social and health-related services by providing:

  •  Full time nursing services, such as care management, supplemental nutrition, continence care, medication administration and management. 

  •  Nutritional meals and snacks that meet individual dietary requirements.

  •  Dignified assistance and support with activities of daily living, including personal care.

  •  A sense of community by promoting relationship-building

Transportation to and from the center is available through DART.


Memory Care Services

Memory Care Services are for adults in the early or mid-stages of Alzheimer’s or other dementia related conditions. 

These services provide a safe, caring environment for participants, while allowing spouses and other family caregivers a daily respite from the challenges of providing care for another adult.


Funding

We accept various funding sources including Medicaid, Veteran’s Administration, and self-pay.   Fee assistance may be available to qualifying applicants.
