Basic Responsibilities:

The Adult Day Care Site Supervisor is responsible for managing the service delivery of client care within the Adult Day Care program.

ESSENTIAL DUTIES:
•    Provide nursing care to clients as needed per their plan of care.
•    Supervise staff and volunteers to ensure the highest quality of care and safety is provided.
•    Provide oversight in the creation of the activities calendar to ensure it meets the needs of the clients.
•    Complete employee orientation, training and complete annual evaluations.
•    Ensure that all necessary requests/reports are completed and submitted within established time frames.
•    Ensure appropriate staffing is maintained and ratios are followed.
•    Perform client intake and admissions at Easterseals or within client homes
•    Provide the RN with objective and subjective data to assess the needs of the client and assist the RN in the creation of the plan of care at intake and ongoing as needed.
•    Coordinate with family/guardians/clients/case managers, and/or other significant stakeholders to schedule special team meetings and plan of care meetings as required by regulations or the needs of the client.
•    Complete all paperwork, billing tasks, and record-keeping in a timely manner.
•    Perform nursing functions including medication administration in accordance with standards of licensure for the profession.  
•    Facilitate group interaction to assure continuity of care for all participants enrolled.
•    Adhere to the standards of conduct, established Code of Ethics, confidentiality, privacy, and security of protected health information, and all policies and procedures of Easterseals.
•    Attend mandatory staff meetings, in-service training, and competency sessions as required for the position.
•    Perform other related duties as directed by the RN.

Qualifications:

•    Current, valid, unrestricted nursing license.
•    Minimum of 2 years of nursing experience in a supervised setting.   
•    Experience in the delivery of services for adults and children.  
•    Experience with regulatory and accreditation standards associated with service delivery.
•    Must have a valid driver’s license and reliable transportation.
•    Must have proof of auto insurance.
•    A Criminal background check must meet any requirements of the State of Ohio licensure for involvement in clinical programs.

•    Knowledge of nursing services and ethical code of conduct associated with the State Board of Nursing.
•    Knowledge of regulatory standards associated with accreditation and service delivery such as Medicare/Medicaid, VA, PASSPORT, Title III, Caregiver Support, DODD, Grants, CARF, etc.
•    Knowledge of the roles, responsibilities, and duties of team members.
•    Ability to work independently, exercise creativity and sound judgment, be attentive to detail, and maintain positive interactions.         
•    Experience with electronic medical record.
•    Ability to manage multiple and simultaneous responsibilities and prioritize scheduling of work.
•    Ability to maintain confidentiality of all medical, financial, and legal information.
•    Ability to complete work assignments accurately and in a timely manner.
•    Ability to communicate effectively, both orally and in written format.
•    Ability to handle conflict and recommend resolutions in a professional manner.
•    Displays a high level of motivation and a self-imposed standard of excellence.

EQUIPMENT USED/MACHINERY REQUIRED:
•    General office equipment including computers and applicable software programs.  
•    Medical equipment utilized in the delivery of client care
•    Operate a motor vehicle safely.  

PHYSICAL DEMANDS:
•    Full range of body motion including handling and lifting of clients.
•    May be required to lift, carry, push, or pull up to 50 pounds to assist in the delivery of client care.
•    Frequently requires sitting, standing, walking, bending, squatting, kneeling, and reaching.
•    Potential exposure to blood and body fluids, communicable diseases, chemicals, and repetitive motions.
•    Occasionally exposed to loud/noisy environments

FLSA Status: Exempt $40,000 - $45,000 per year Benefits: PTO, Paid Holidays, Sick Time, Medical, Company Paid Life/Short-Term Disability, Voluntary Benefits, and 401(k) with match. Drug-Free Workplace/EOE
