Summary:

Easterseals of Northeast Ohio is looking for an energetic, compassionate team player to serve our clients and help make a difference. The right candidate would work approximately 19 hours weekly as needed. This position will assist clients in the Adult Day Center between the Mahoning and Columbiana locations. The right candidate for this position will be required but not limited to:

Basic Responsibilities:

Perform tasks and provide necessary aid to meet the physical, personal, and social needs of clients of all ages as defined within the Individualized Plan of Care. Staff members will work cooperatively and professionally as a part of the team and support the person-centered philosophy to enhance or improve the quality of life of the persons served.

ESSENTIAL DUTIES & RESPONSIBILITIES: (Other duties may be assigned)

•    Assist or prompt consumers to safely perform social, physical, educational, and recreational activities in cooperation with the treatment team.
•    Assist client when arriving and departing the facility through means of private, public, or Easterseals transportation.
•    Assist client to navigate and interact safely on community outings including monitoring safety during transportation on vehicles.
•    Assist client with walking and transferring manually and with the use of a mechanical lift as needed, bathing, eating, and maintenance of personal hygiene as required by the individual needs and by the plan of care.
•    Assist client with routine bodily functions, toileting, incontinence, and peri-care, as well as catheter care.
•    Observe and/or prompt the client through verbal, gestural, and physical cues.
•    Effectively communicate changes in client status or information that was observed or heard to the supervisor and ensure that appropriate documentation is maintained according to program standards.
•    At the instruction of the Nurse, provide care to support the client or client’s plan of care and goals.
•    Utilize and follow the activity calendar and lead participants in activities to maintain continuous implementation of the daily schedule and support client goals.
•    Prepare and maintain materials and equipment for each day’s activities to ensure effective flow of schedule.
•    Prepare food and serve food by USDA guidelines.
•    Protect the health and safety of clients, coworkers, and others through the maintenance of a clean and sanitary environment.
•    Participate and complete educational in-service training and competencies as required for the position.
•    Submit documentation of job-related training, licensure, and medical testing as required.

NON-ESSENTIAL DUTIES – None identified as the primary purpose of the position is to provide support and assistance to the clients served in the HCBS programs.

Qualifications:

To perform this job successfully, an individual must carry out each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. •    Must be 18 years of age or older.
•    Must have a High School diploma or equivalent.
•    Successful completion of a vocational program in the health or human service field, State-Tested Nursing Assistant, or Certified Nurse Aide certification is preferred.
•    A minimum of two years of employment in a supervised position to provide personal care and activities or assist with activities is preferred.
•    Must be able to meet agency requirements for criminal background checks, which meets the requirements for Direct Care personnel within the state of Ohio.
•    Drug and Alcohol testing per company policy and safety program.

•    Must exercise sound judgment in making decisions, solving problems, following the chain of command in difficult situations, and acting accordingly.

EQUIPMENT USED/MACHINERY REQUIRED TO OPERATE:
•    General Office Equipment and applicable software.
•    Food securement devices and coolers.
•    Wheelchair and passenger securement/safety devices.

PHYSICAL DEMANDS
•    Frequent standing, walking, bending, stooping, and twisting.
•    Frequent pushing/pulling/maneuvering of mobility devices such as wheelchairs.
•    Must be capable of lifting a minimum of 30 lbs. without difficulty.
•    Exposed to hot or cold temperatures.
