Easterseals of Northeast Ohio is looking for an energetic and compassionate full-time LPN to serve our clients and help make a difference at our Lisbon location. No evenings or weekends.

Basic Responsibilities:

The LPN is responsible for providing nursing care for the clients in the Easterseals Adult Day Care.

ESSENTIAL DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:
•    Provide direct nursing care to clients in accordance with standards of licensure.
•    Assist in the facilitation of group activities as needed.
•    Assist the team with various tasks to ensure all client needs are met including but not limited to meal preparation, feeding, toileting, and transferring.
•    Assist with the client intake and admissions process as needed.
•    In collaboration with the RN, create and update care plans.
•    Assist with the implementation of care plan goals.
•    Assist the team with various tasks to ensure all client needs are met including but not limited to meal preparation, feeding, toileting, and transferring.
•    Assist with quarterly data collection and with organizing documents for audits as needed.
•    Attend care conferences.
•    Attending mandatory staff meetings and in-service training, and competency sessions as required for the position.
•    Maintains updated knowledge of regulatory standards as it relates to Medicare, CARF, ODA, ODH, VA, and other regulatory bodies as they apply to all Home and Community-Based Services.
•    Adhere to the standards of conduct, established Code of Ethics, confidentiality, privacy, and security of protected health information, and all policies and procedures of Easterseals.
•    Perform other related duties as assigne

Qualifications:

To perform this job successfully, an individual must be able to carry out each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.  Reasonable accommodation may be provided to enable individuals with disabilities to perform essential functions.

•    Current, valid, unrestricted practical nursing licensure.
•    Experience in the delivery and operation of adult services preferred.  
•    Must have a valid driver’s license and reliable transportation.
•    Must have proof of auto insurance.
•    A criminal background check must meet any requirements of the State of Ohio licensure for involvement in clinical programs.

KNOWLEDGE, SKILLS, ABILITIES AND OTHER ATTRIBUTES:
•    Knowledge of nursing services and ethical code of conduct associated with the State Board of Nursing.
•    Knowledge of regulatory requirements associated with state, federal, and local laws as well as CARF standards
•    Displays a high level of motivation and a self-imposed standard of excellence.
•    Ability to determine appropriate course of action in complex situations.
•    Ability to work independently, exercise creativity and sound judgment, be attentive to detail, and maintain positive interactions.         
•    Experience with software programs for client scheduling, care plans, etc.
•    Ability to manage multiple and simultaneous responsibilities and prioritize the scheduling of work.
•    Ability to communicate effectively, both orally and in written format.
•    Ability to handle conflicts and recommend resolutions professionally.

EQUIPMENT USED:
•    General office equipment including computers and applicable software programs.  
•    Medical equipment to be utilized in the delivery of client care.
•    Operate a motor vehicle safely.

PHYSICAL DEMANDS:
•    Frequently requires sitting, standing, walking, bending, squatting, kneeling, and reaching.
•    Occasionally exposed to loud/noisy environments.
•    Full range of body motion, including handling and lifting of clients.
•    May be required to lift, carry, push, or pull up to 50 pounds to assist in the delivery of client care
