Summary:

Easterseals of Northeast Ohio is looking for an energetic and compassionate LPN to work in our Adult Day Care to serve our clients and help to make a difference. This position is per diem (PRN) and scheduled to work 14-19 hours per week. No evenings or weekends.

Basic Responsibilities:

The HCBS LPN is responsible for providing nursing care for the clients in the Easterseals Adult Day Care. Responsible to internal and external customers for courteous, accurate, timely service.

ESSENTIAL DUTIES (FUNCTIONS) INCLUDE BUT NOT LIMITED TO THE FOLLOWING:
•    Maintains updated knowledge of regulatory standards as it relates to Medicare CARF, ODA, ODH, VA, and other regulatory bodies as they apply to all Home and Community-Based Services.
•    In coordination with the RN, ensure that all necessary documentation is completed and submitted within established time frames.
•    Provides necessary nursing care to clients of Easterseals.
•    Directs, provides, and/or assists with the client’s personal care and implementation of care plan goals.
•    Performs client intake and admissions protocol at Easterseals.
•    In collaboration with the RN, initiates and develops care plans.
•    Provides all necessary training for staff within the Easterseals Adult Day Care.
•    In collaboration with the RN, performs consumer reassessments and re-development or modification of the plan of care in accordance with program guidelines or as necessitated by change in consumer status.
•    Perform nursing functions including medication administration in accordance with standards of licensure for the profession. Provides direct care to comply with ratio and client needs.  
•    Facilitate group interaction to assure continuity of care for all enrolled participants.
•    Maintain effective communication with community agencies, organizations, and referral sources to facilitate client service and to provide education and support to consumers and families.
•    Coordinate and attend team conferences with regulatory Case Managers as required (i.e. Care Star, PASSPORT, etc.)
•    Adhere to the standards of conduct, established Code of Ethics, confidentiality, privacy, and security of protected health information, and all policies and procedures of Easterseals.
•    Attending mandatory staff meetings, in-service training, and competency sessions as required for the position.
•    Submit documentation of job-related training, licensure, medical testing, and insurance as required.
•    Support RN and promote team-style coordination of scheduling and ensuring accuracy, timeliness, and the completion of all work performed.

NON-ESSENTIAL DUTIES:
•    Provide direct care and assistance to consumers in attendance as necessary to maintain continuity of service and maintenance of designated ratios.
•    Performs other related duties as assigned by the RN.

Qualifications:

To perform this job successfully, an individual must be able to carry out each essential duty satisfactorily. •    Current, valid, unrestricted practical nursing licensure;
•    Experience in the delivery and operation of adult services preferred.  
•    Must have a valid driver’s license and reliable transportation.
•    Must have proof of auto insurance.
•    A Criminal Background Check must meet any requirements of the State of Ohio licensure for involvement in clinical programs.

KNOWLEDGE, SKILLS, ABILITIES AND OTHER ATTRIBUTES:
•    Knowledge of nursing services and ethical code of conduct associated with the State Board of Nursing.
•    Knowledge of regulatory standards associated with accreditation and service delivery such as Medicare/Medicaid, VA, PASSPORT, Title III, Caregiver Support, DODD, Grants, CARF, etc. Preferred.
•    Knowledge of the roles, responsibilities, and duties of team members; accurately assesses the impact upon others of role performance; and supports and promotes organizational decisions, policies, programs, and initiatives.
•    Organizational and interpersonal skills.
•    Displays a high level of motivation and a self-imposed standard of excellence.
•    Ability to determine appropriate course of action in complex situations.
•    Ability to work independently, exercise creativity and sound judgment, be attentive to detail, and maintain positive interactions.         
•    Experience with software programs for client scheduling, care plans, etc.…
•    Ability to manage multiple and simultaneous responsibilities and prioritize scheduling of work
•    Ability to maintain confidentiality of all medical, financial, and legal information
•    Ability to complete work assignments accurately and in a timely manner
•    Ability to communicate effectively, both orally and in written format
•    Ability to handle conflict and recommend resolutions in a professional manner

EQUIPMENT USED/MACHINERY REQUIRED TO OPERATE:
•    General Office Equipment including computers and applicable software programs.  
•    Medical equipment utilized in the delivery of client care
•    Operate a motor vehicle safely

PHYSICAL DEMANDS:
•    Full range of body motion including handling and lifting of clients.
•    May be required to lift, carry, push or pull up to 50 pounds in order to assist in the delivery of client care.
•    Frequently requires sitting, standing, walking, bending, squatting, kneeling and reaching.
•    Potential exposure to blood and body fluids, communicable disease, chemicals, and repetitive motions.
•    Required to assist clients with duties based on the individual Plan of Care while attending Adult Day Care and participating in community outings.
•    Occasionally exposed to loud/noisy environments.
