Care Coordination

Prime Care Coordination is an official Health Home/Care Coordination Organization (HH/CCO), for Individuals with Intellectual and Developmental Disabilities. It was created when a group of experienced not-for-profit agencies came together to form a network to make sure people in Upstate New York could get the most out of New York State’s Office for People With Developmental Disabilities’ (OPWDD) new People First Care Coordination approach.
