Know Your Rights

Your Right to Choose

  • You have the right to change providers or choose to discontinue services at any point in treatment. 
  • You have the right to be informed of details about your provider and treatment choices. 

Your Right to Privacy

  • You have the right to privacy and protection of your personal treatment information. 
  • You have the right to obtain copies of your treatment record. 
  • There are some exceptions to the privacy rules, which you have the right to be informed about. Your provider may break confidentiality if someone is hurting you, neglecting your basic needs, or if you have a plan to harm yourself or others.

Your Right to Be Accepted 

  • You have the right to be treated with dignity and respect regardless of your race, color, sex, sexual orientation/preferences, gender identity, religion, disability, age, or nationality/ethnic origin.

Your Right to Have a Say

  • You have the right to participate in your treatment plan and understand what to expect in treatment. 
  • You have the right to be informed of treatment options and alternatives. 
  • You have the right to share if you are dissatisfied with your treatment.
  • You have the right to refuse, discontinue or delay treatment at any time.
