WHAT IS THE PERSONAL HEALTHCARE PORTAL?
It is an online tool that will allow you to view your personal healthcare information online 24 hours a day, 7 days a week using a secure and private website. This website is made possible through Oakland Community Health Network.
HOW TO GET STARTED
- Tell your case manager or clinician that you are interested in the portal.
- For your privacy they will create a personal identification number (PIN) for you and give you Instructions on how to create a user account.
- Go to www.MyCEHR.com to access the portal.
