Title VI

It is the policy of Easterseals Midwest that no person shall be excluded from participation in, be denied the benefits of, or be subjected to discrimination in any program or activity of the organization. All complaints must be filed within 180 days following the date of the alleged discriminatory occurrence and should contain as much detailed information about the alleged discrimination as possible.

In addition to the complaint process at Easterseals Midwest, complaints may be filed directly with the Federal Transit Administration, Office of Civil Rights.

Accessibility – ADA  

Easterseals Midwest will engage in an interactive process in order to provide reasonable accommodations to practices and procedures to ensure that programs are accessible to all individuals.   

If you have a complaint about the accessibility of our locations, programs, equipment or believe you have been discriminated against based on a disability please complete a complaint form to initiate our review process. The complaint should be in writing and contain information about the alleged discrimination such as name, address, phone number of complainant and location, date and description of the concern/issue encountered.

The complaint should be submitted as soon as possible, but no later than 180 calendar days after the alleged violation. Complaints may be submitted directly to:
